Service Request Acknowledgment and Shipping Instructions

Thank you for submitting your service and repair request. We have successfully received your request, and a copy has been sent to the email address you provided.

What to Do Next:

  1. Open the email we sent you from CAROLINA DVE CENTER (noreply@smartwaiver.com.). Click the
    Verify Confirm Email Address  button to verify your email address is correct. Once you verify your email address a copy of the Service Request is emailed to you.
  2. Print the Service Request Email: Please print the service request email and include a copy inside the box when mailing your items to us.If you do not have access to a printer, please include a handwritten note with the following information:
    • Your Name
    • Address
    • Phone Number
    • Email Address
  3. Mailing Instructions: Kindly send your package to the following address:

Once we receive your scuba gear, we will contact you to confirm the details of the service work to be performed. You will also receive an update on the estimated timeline for completion.
